POSITION OVERVIEW :

These positions are located at Sparta and Chester CILA (Community Integrated Living Arrangement) housing. You will be assisting developmentally disabled adults and helping them with day-to-day functions. This position may require working overnights.
Job Duties :
- Assist in the development, review and modification of Client Centered Individualized Treatment plans for all assigned clients by identifying individual’s strengths, needs, and goals.
- Ensures accurate implementation of Person Centered Plan and documentation of outcomes of implementation strategies accurately and in a timely fashion.
- Assist with basic personal hygiene (such as tooth-brushing, toileting, bathing, clothing), grooming, feeding, ambulation, medical monitoring, and health care related tasks.
- Assist in the monitoring of the overall functioning of each assigned client and communicating any changes in the level of functioning and or medical condition of clients to the appropriate staff.
- Assist in the development and implementation of behavior plans, completion of appropriate documentation (tracking sheets, behavior monitoring forms, and shift notes). Assists in crisis situations as needed.
- Responsible for safeguarding client funds, assisting in client budgeting and completing appropriate financial documentation.
- Responsible for medication administration. Must maintain Approved Medication Administration status and adhere to policies and procedures relevant to medication administration.
- Provide assistance with individual’s emotional issues and provide support and solutions.
- Provide habilitative and rehabilitative services and educational instruction to clients in the areas of daily living skills, social and recreational skills, money management, and health and medication management.
- Responsible for the development of balanced menus based on dietary restrictions, schedule planning for household chores, outdoor chores, meal preparation and community/social/recreational planning. Shares responsibility of shopping in the community for needed/necessary items such as groceries, household supplies and completed required documentation.
- Responsible for developing opportunities for individuals to have increased opportunities for interacting with members of the community and living in a more integrated environment.
- Acts as an advocate for individuals and assist individuals in self-advocacy regard to community relations and the betterment of their lives.
- Performs housekeeping duties such as vacuuming, shopping, laundry, and mopping as needed.
- Drives agency vehicle and provides transportation for individuals in the program to and from appointments, community integration activities, and other general needs.
- Perform related or other duties as assigned by manager.
- Maintain compliance with all ComWell policies and procedures.
Job Requirements :
- Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers.
- Computer skills (MS Office – Word, Excel and Outlook).
- Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.
- Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
- High school diploma or GED required, DSP certification within 120 days of hire.
- Valid driver’s license, able to pass a DOT physical.
- Must have reliable phone or contact source.

Pay : $18 – $19.25 per hour
Expected hours : Maximum 60 hours per month
