Job Title : Employment Support Services Coordinator

Purpose of Position : In keeping with the Mission/Vision/Values of ComWell, the Employment Support Services Coordinator is responsible for the growth and development of employment services for clients using the evidence-based practice, Individual Placement Support (IPS).

 

Essential Duties and Responsibilities :

1. 6. Have a basic understanding of the impact trauma has on a person’s life. Be aware of the signs, symptoms and special needs a trauma survivor may have.

7. Have an awareness of and commitment to self-care practices that prevent or lessen the impact of secondary traumatization with staff and self.

8. Participate in and recognize role in Performance and Quality Improvement (PQI) initiatives. Have a general awareness on the importance of quality improvement throughout the organization. Identify opportunities for improvement through data, observation of operations or consultation with staff, residents, families or other stakeholders. Identification and input into opportunities for improvement is strongly encouraged.

9. Work alongside Manager and Director of Developmental Disabilities to implement new employment program in line with CCBHC directives and IPS standards to meet the needs of the clients within the community.

10. Ensures good program outcomes by providing training, supervision, and by working side-by-side with employment specialists (field mentoring). Evaluate program performance and recommend improvements based on participant feedback and outcome data.

11. Develops expectations for employment specialists regarding employer contacts, number of job starts each quarer and parentage of people employed on each caseload.

12. Conducts weekly group and/or supervision following the principles of IPS supported employment.

13. Arranges for IPS supported employment fidelity reviews. Assists with the development of fidelity action plans based on recommendations from the fidelity report

14. Responsible for small caseload including completion of the state approved assessment instrument, IM+CANS at intake and at required intervals and related IATP’s. Identify employment strengths, goals, and skills of individuals and match with roles that suit skills and experience. Provide career planning to support their decision-making process, including career counseling, assessment, job search, soft skill development, and enrollment into on-the-job training.

15. Supervise clients to ensure that work is performed according to prescribed methods and meets established standards. Explains and demonstrates tasks to resolve work related problems or difficulties.

16. Scheduling and overseeing follow-up support to assist client with maintaining their employment.

17. Participate in recruitment strategies to increase opportunities for competitive employment for clients. This includes contacting prospective employers, learning about job requirements, explaining the program and describing the job seeker to the potential employer to find the best match and maintaining communication with employers to mitigate any employment related challenges or concerns.

18. Build on existing relationship with Department of Rehabilitation Services (DRS). This includes understanding DORS through training participation and the referral and contracting process.

19. Responsible for gathering and tracking employment data related to the funding source.

20. Build partnerships with local employers, educational institutions, training providers, and community organizations.

21. Driving is an essential function of this position and the ability to perform driving responsibilities must be maintained throughout employment in this position.

22. Perform related or other duties as assigned by manager.

23. Maintain compliance with all ComWell policies and procedures.

 

Education and/or Work Experience Requirements :

  1. Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers.
  2. Excellent computer proficiency (MS Office – Word, Excel and Outlook). Excellent mathematical and mechanical skills required.
  3. Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.
  4. Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices
  5. Minimum of Bachelor’s Degree in social work or other human service related field.
  6. Must pass healthcare workers background check, CANTS and criminal record check.
  7. Must complete IPS training within first year of employment.
  8. Valid driver’s license and proof of valid automobile insurance.
  9. Human service background preferred.

 

Physical Requirements :

  1. Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
  2. Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
  3. Must be able to lift up to 50 pounds, pull, bend, and stoop, stand for significant periods of time.

 

Job Type : Full-Time

 

Pay : $24.10 per hour
