Job Title : Clinical Outcomes Specialist

Reports to : Director of Quality Assurance
Purpose of Position : In keeping with the Mission/Vision/Values of ComWell, the Clinical Outcomes Specialist improves the health outcomes of a group by monitoring and identifying data trends within the electronic healthcare record. This position also monitors individual patients within that group; usually based on medical conditions, age, and location.
Primary purpose is to complete tasks like aggregating data to provide a complete clinical picture of individuals and/or groups of individuals. By using data like these, the providers then can easily track the results of care. This position will coordinate the planning, oversight and completion of projects and complex tasks needed to ensure the successful performance in value-based contracts and compliance with contract requirements. This includes development and maintenance of tracking tools, project plans, and status reports; as well as, responsibility for basic data analysis, collection, submission and research.
Essential Duties and Responsibilities :
1. 6. Have a thorough understanding on the impact of trauma on a person’s life. Understand the signs and symptoms of trauma in clients and families and be competent in delivering trauma informed interventions that lessen the symptoms associated with trauma while improving the quality of life for clients.
7. Have an awareness of and commitment to self-care practices that prevent or lessen the impact of secondary traumatization with staff and self.
8. Participate in and recognize role in Performance and Quality Improvement (PQI) initiatives. Have a general awareness on the importance of quality improvement throughout the organization. Identify opportunities for improvement through data, observation of operations or consultation with staff, residents, families or other stakeholders. Identification and input into opportunities for improvement is strongly encouraged.
9. In-depth, specialized knowledge of the CCBHC grant and related reportables and deliverables. This will also include participation in trainings, webinars, and work groups related to CCBHC.
10. Coordination with CCBHC Project Director, CCBHC Evaluator and other members of the ComWell leadership team to ensure all CCBHC deadlines for deliverables and reporting are met; this will include preparation of reports and documents for review by the Project Director or other members of the team as applicable.
11. Serves as the liaison to work on developing and implementing environmental strategies to improve population health, including direct linkage to services or education.
12. Collaborate with managers and clinicians to ensure timely completion of outcome measures and assessments.
13. Train staff on best practices related to outcome documentation and interpretation.
14. Identifies and/or strengthens partnership with providers or organizations to help promote capabilities to treat the whole person.
15. Prepares various reports to present findings and to highlight successes and opportunities for improvement or intervention.
16. Incorporate continuous process improvement into daily work using critical thinking and problem-solving skills to identify opportunities for improvement while promoting engagement of stakeholders in solution development.
17. Utilize solid business planning principles to develop systems and processes that produce sustainable results and support ongoing operations.
Education and/or Work Experience Requirements :
- Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers.
- Excellent computer proficiency (MS Office – Word, Excel and Outlook and Electronic Healthcare Records)
- Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.
- Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
- Bachelor’s degree in Psychology, Social Work, Public Health, Data Analytics, or related field. Two years of relevant administrative and program coordination experience preferred.
- Knowledge and ability to: collaborate with a diverse group of community partners, experience planning, implementing, and evaluating public health programs.
Physical Requirements :
- Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
Job Type & Schedule : Full-Time; 37.5 hours per week, 7:30am – 3:30pm, Monday – Friday
Pay : $22.00 per hour
