Job Title : Authorized Direct Support Professional – Community Integrated Living Arrangement (CILA)

Purpose of Position : In keeping with the Mission/Vision/Values of ComWell, a Counselor I provides services to individuals seeking treatment with a behavioral health diagnosis. This position works with individuals to assess needs and identify treatment goals and outcomes achievable through counseling and advocacy services. All services should be trauma informed and reflect person and family centered principles of recovery through coordination of care with other involved service providers.

Essential Duties and Responsibilities :
1. 6. Have a basic understanding of the impact trauma has on a person’s life. Be aware of the signs, symptoms and special needs a trauma survivor may have.

7. Have an understanding and commitment to self-care practices to prevent or lessen the impact of secondary traumatization with staff and self.

8. Participate in and recognize role in Performance and Quality Improvement (PQI) initiatives. Have general awareness on the importance of quality improvement throughout the organization. Identify opportunities for improvement through data, observation of operations or consultation with staff, persons served or other stakeholders. Identification and input into opportunities for improvement is strongly encouraged.

9. Assist in the development, review and modification of Client Centered Individualized Treatment plans for all assigned clients by identifying individual’s strengths, needs, and goals.

10. Ensures accurate implementation of Person Centered Plan and documentation of outcomes of implementation strategies accurately and in a timely fashion.

11. Assist with basic personal hygiene (such as tooth-brushing, toileting, bathing, clothing), grooming, feeding, ambulation, medical monitoring, and health care related tasks.

12. Assist in the monitoring of the overall functioning of each assigned client and communicating any changes in the level of functioning and or medical condition of clients to the appropriate staff.

13. Assist in the development and implementation of behavior plans, completion of appropriate documentation (tracking sheets, behavior monitoring forms, and shift notes). Assists in crisis situations as needed.

14. Responsible for safeguarding client funds, assisting in client budgeting and completing appropriate financial documentation.

15. Responsible for medication administration. Must maintain Approved Medication Administration status and adhere to policies and procedures relevant to medication administration.

16. Provide assistance with individual’s emotional issues and provide support and solutions.

17. Provide habilitative and rehabilitative services and educational instruction to clients in the areas of daily living skills, social and recreational skills, money management, and health and medication management.

18. Responsible for the development of balanced menus based on dietary restrictions, schedule planning for household chores, outdoor chores, meal preparation and community/social/recreational planning. Shares responsibility of shopping in the community for needed/necessary items such as groceries, household supplies and completed required documentation.

19. Responsible for developing opportunities for individuals to have increased opportunities for interacting with members of the community and living in a more integrated environment.

20. Acts as an advocate for individuals and assist individuals in self-advocacy regard to community relations and the betterment of their lives.

21. To participate in in-service training and community presentations as assigned to provide education and consultation for community organizations.

22. Perform related or other duties as assigned by manager.

23. Maintain compliance with all ComWell policies and procedures.

 

Education and/or Work Experience Requirements :
1. Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers.
2. Driver’s license and proof of valid automobile insurance.
3. Must meet expectations required by CANTS check, criminal record and Health Care Workers Background Check
4. Excellent computer proficiency (MS Office – Word, Excel and Outlook).
5. Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.
6. Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
7. Bachelor’s Degree in a Social Service Program required.

 

Physical Requirements :
1. Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
2. Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.

Schedule : 40 hour work week, every other weekend
