Job Title : Care Coordinator – Hospital Specialist

Purpose of Position : In keeping with the Mission/Vision/Values of ComWell, the Care Coordinator II – Hospital Specialist provides services to individuals with a behavioral health diagnosis seeking treatment. This position promotes improved quality of life and functioning, and the health, safety and welfare of persons served through a person centered, trauma informed lens. Services are to reflect the principles of recovery, active participation in treatment, and community integration within the limits of established and approved policies, standards and practices of ComWell and all regulatory agencies. Services may be provided within a residential setting, community setting, an individual’s home or an area hospital.
Essential Duties and Responsibilities :
1. 6. Have a thorough understanding on the impact of trauma on a person’s life. Understand the signs and symptoms of trauma in clients and families and be competent in delivering trauma informed interventions that lessen the symptoms associated with trauma while improving the quality of life for clients.
7. Have an awareness of and commitment to self-care practices that prevent or lessen the impact of secondary traumatization with staff and self.
8. Participate in and recognize role in Performance and Quality Improvement (PQI) initiatives. Have a general awareness on the importance of quality improvement throughout the organization. Identify opportunities for improvement through data, observation of operations or consultation with staff, residents, families or other stakeholders. Identification and input into opportunities for improvement is strongly encouraged.
9. Ensure quality service delivery to all clients based on the rules governing the program and service being offered. Mental Health Services follows Rule 140, Substance Use Services are guided by Rule 2060 and both services must meet Council on Accreditation (COA) standards. All services delivered should also meet other rules, regulations, laws and ethical codes related to operations.
10. Assist in the development of a service plan for all assigned clients and client related service responsibilities. Monitor the overall level of functioning of each assigned client. Notify supervisor when a client is experiencing a decline in level of functioning.
11. Provide structured services that are goal directed and clearly defined as medically necessary through the client’s assessment and treatment plan and focused on improving adaptive functioning deficits. This may be done through a series of sessions with modules addressing different components of functioning such as, but not limited to, social competency, anger management, problem-solving/decision-making, or the ability to live independently.
12. Assist in the development and scheduling of client service activities related to medically necessary services as defined through the client’s assessment and treatment plan. Make scheduled contacts with clients in their residence and other scheduled visits in the community.
13. Follow Treatment Plan for each client, complete documentation of program goals and progress notes in accordance with Medicaid requirements, and other data that is required per program and agency guidelines.
14. Provide direct interactions with the client or on behalf of the client with a member of the client’s family, with such interactions intended to result in improving or maintaining the client’s ability to function in a variety of interpersonal situations, including in the family, school or community. This may include, but is not limited to: interpersonal communication, dating and sexual appropriateness, peer or sibling interactions, self-regulating behavior, problem-solving skills, parent/child communication and interaction, self-care and hygiene, money management, health care and general care of their household.
15. Assist with coordination, support and advocacy for clients who need multiple services and require assistance in gaining access to and in using mental health, social, vocational, educational, child welfare, and other community services and resources. This may include the ICG program, applying for public entitlements, locating housing or obtaining medical and dental care.
16. Provide client-specific advocacy and assistance in problem solving/resolution to assist the client in building community support and family support systems.
17. Provide client care in accordance with Hours of Care requirements based on individual standards.
18. Transport clients to scheduled appointments and to other service environments in efforts to enhance personal self-care, household management and social functioning. Driving is an essential function of this position. Examples of transporting clients includes but is not limited to: medical appointments, home visits, social and recreational activities, grocery and personal shopping.
19. The specialty responsibilities for the Care Coordinator working within Sparta Community Hospital or Chester Memorial Hospital also include the following:
- Call all discharged inpatient and observation patients that have high risk for readmission by LACE and then document in the patient care notes.
- May consult with the ComWell Social Worker for all Social Service needs for inpatient, observation, outpatient and swing bed patients.
- Attend Care conference regularly and completes the documentation and follow up required.
- May collaborate with Infusion Center, Emergency Department and Outpatient clinics to improve patient outcomes through care coordination and resources.
- Communicate regularly with the Care Coordination Manager and/or Chief Nursing Officer.
- Maintain data on patients seen, calls made, and care conferences attended.
- Presents PQI data annually in collaboration with the Care Coordination Manager and ComWell.

20. Attend meetings, staffing’s, and training activities as needed to improve knowledge and skills relevant to role.
21. Crisis back up as needed per manager.
22. Perform related or other duties as assigned by manager.
23. Maintain compliance with all ComWell policies and procedures.
24. Maintain compliance with, or display consistent efforts to achieve, expected Hours of Care requirements.
Education and/or Work Experience Requirements :
- Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers.
- Community collaboration experience preferred.
- Driver’s license and proof of valid automobile insurance.
- Must meet expectations required by CANTS check, criminal record and Health Care Workers Background Check
- Excellent computer proficiency (MS Office – Word, Excel and Outlook).
- Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.
- Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
- Bachelor’s degree preferred or five years relevant work experience.
Physical Requirements :
- Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
Job Type & Schedule : Full-Time; 37.5 hours per week, 7:30am – 3:30pm, Monday – Friday
