Job Title : Community Integrated Living Arrangement (CILA) Program Manager

Purpose of Position : In keeping with the Mission/Vision/Values of ComWell, this position is primarily responsible for the program oversight of intermittent, Family and 24-hour Community Integrated Living Arrangement (CILA) programs for individuals with Developmental Disabilities. This position is directly responsible for the development and management of a residential location providing 24-hour care to individuals who have a diagnosis of developmental disabilities. This position provides service coordination, advocacy, medical care and oversight and therapeutic interventions to individuals served; additionally, they work with family members, guardian, and other providers for the well-being of the individuals served. Responsibilities include ensuring all aspects of the program comply with the state regulations and agency policies through a trauma informed lens.
Essential Duties and Responsibilities :
1. 7. Have an awareness of and commitment to self-care practices that prevent or lessen the impact of secondary traumatization with staff and self.
8. Participate in and recognize role in Performance and Quality Improvement (PQI) initiatives. Have a general awareness on the importance of quality improvement throughout the organization. Identify opportunities for improvement through data, observation of operations or consultation with staff, residents, families or other stakeholders. Identification and input into opportunities for improvement is strongly encouraged.
9. Responsible for operating the program in a manner that protects and promotes the rights of individuals, reflects person centered planning and is reflective of person first language.
10. Responsible for the recruitment, training, scheduling, and supervision of employees working within the residential and intermittent programs Responsible for ensuring that all DHS/DSP training is completed, documented, and submitted to Human Resources for record keeping and reimbursement; including monitoring the medication training and retraining annually to ensure compliance with Adm. Rule 116, Rule 115,and OIG as well as any other applicable trainings to maintain compliance with funding sources.
11. Facilitate medication administration training to all DD residential staff, this includes administration of the TABE test to ensure individuals meet the minimum requirement to complete medication training.
12. Responsible for monitoring all medical and psychiatric needs of the clients; including monitoring medication changes, physician orders, lab orders, yearly and as needed medical appointments; ensuring that all orders/correspondence are documented and relayed to staff, guardians, and clients as needed.
13. Monitoring the electronic medication system, discontinuing, approving, and assuring correct medications are available on a daily basis.
14. Supervise direct care staff in the administration of medications as delegated. Check for medication errors on a regular basis, complete medication error reports and determine plan of corrections for staff with medication errors. Provide medical supervision/consultation to residential staff.
15. Provide an initial, annual and as needed nursing assessment for DD CILA residents within the time restraints noted in Adm Rule 116.
16. Responsible for entering client data including medication changes into the HRST system.
17. Responsible for the implementation, review, and documentation of the person centered service plan for all assigned clients. Is a member of the Interdisciplinary Team and works as a team member with other staff for the implementation of overall person centered planning, this may include goal/program development.
18. Provides case management activities to assigned clients. Functions as a liaison between ComWell and the community, advocating for clients who require assistance in gaining access or maintaining resources available to them.
19. Coordinate with psychiatric team appropriate procedures regarding client medications to ensure compliance with state regulations.
20. Review and revise Adm Rule 116 policy and procedures, ensure compliance with stat regulations and participate in Adm Rule surveys.
21. Responsible for ensuring the overall completeness of the client service record.
22. Complete monthly progress summaries within the time limits established by the program.
23. Responsible for maintaining timely and accurate communication with guardians, family members, and other designated persons per the request of the client.
24. Responsible for the recruitment, screening, and admission process of clients entering the CILA program, including nursing assessments for potential CILA clients.
25. Responsible for managing client funds as acting representative payee, ensuring that accounts remain in good standing while assisting clients in the budgeting their resources.
26. Responsible for ensuring that clients maintain Medicaid, LINC, and other sources of income/benefits by ensuring all required documents are maintained and reports are completed accurately and timely.
27. Assist the Director of Developmental Disabilities with fiscal decisions, such as budget recommendations, billing procedures, requisition and purchasing decisions. Responsible for coordinating and consulting with the Director on activities regarding planning, monitoring and evaluation of program, and assist in developing and evaluating specific program problems, goals or changes. This may include staff, building, and equipment utilization; policy and procedure development.
28. Provide supportive, adaptive, and reality based counselling to assigned clients. Provide crisis consultation to staff in regards to the needs of the clients being served as well as crisis counseling to the clients.
29. Advocates for and is actively involved in the planning and implementation of new or expanded services, including increasing community integration opportunities for clients including but not limited to volunteer work, paid work, increased community recreational opportunities.
30. Responsible for maximizing funding in all areas of operation within the budget constraints that are established annually.
31. Coordinates and consults with various communities, local and state organizations in order to enhance service delivery and provide information regarding ComWell services. Functions as a liaison between ComWell and the community, guardians, PAS/Independent Service Coordination (ISC) agencies and the Office of Rehabilitation
32. Complete DSP and or QIDP training in accordance with State Regulations, possess a valid driver’s license, and maintain acceptable status under the Healthcare Workers Background Check Act.
33. Participates in the overall agency planning and development as a member of the management team, participates in trainings designed to enhance knowledge and skills for the betterment of the program/agency.
34. As Needed, Supervises and coordinates Special Olympics; this may include attending Special Olympic meetings, coordinating practice, advocating for new events to participate. Overnight stays may be required for some events.
35. To work with other Management Team members to integrate and ensure coordinated care between programs to the maximum extent possible when appropriate.
36. Perform related or other duties as assigned by director.
37. Maintain compliance with all ComWell policies and procedures.
Education and/or Work Experience Requirements :
- Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers.
- Excellent computer proficiency (MS Office – Word, Excel and Outlook).
- Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.
- Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
- Current RN or Bachelor degree in Rehabilitation, Counseling, Special Education, Psychology or other related field, plus a minimum of one year of professional work experience in the developmentally disabled/intellectual disabled population.
Physical Requirements :
- Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
- Must be able to occasionally lift and or move, push and or pull up to 50 lbs. or more, able to move frequently, required to stoop, kneel, crouch, bend.
- Ability to pass DOT physical as required in order to transport clients as needed.
Job Type & Schedule : 37.5 hours per week, Monday – Friday
Pay : $48,900 annual salary
