In alignment with ComWell’s Mission, Vision, and Values, the Care Coordinator II provides supportive, person-centered services to individuals with behavioral health diagnoses who are seeking treatment. This role focuses on enhancing quality of life, improving daily functioning, and ensuring the health, safety, and overall well-being of those served.
The Care Coordinator II delivers services through a trauma-informed, recovery-oriented approach that encourages active participation in treatment and promotes community integration. Services are provided in a variety of settings, including residential environments, the community, and clients’ homes, while adhering to all ComWell policies and applicable regulatory standards.
Key Responsibilities:
- Provide person-centered care coordination to individuals with behavioral health needs
- Support individuals in achieving treatment goals and improving overall functioning
- Utilize trauma-informed and recovery-oriented practices in all interactions
- Encourage and promote community integration and independence
- Coordinate services and collaborate with internal teams and external providers
- Maintain accurate documentation in compliance with organizational and regulatory requirements
- Ensure services are delivered in accordance with ComWell policies and procedures
Qualifications:

Bachelor’s degree in a human services field required; Master’s degree preferred

Relevant experience in behavioral health or care coordination(MHP)
- Strong communication, organizational, and interpersonal skills
- Ability to work independently and as part of a team

Commitment to person-centered, trauma-informed care

Must meet expectations required by CANTS check, criminal record, and Health Care Workers Background Check
Pay: $22.31 – $25.08 per hour, commensurate with experience and education
